Employee Evaluation Form
The employee evaluation form is a valuable tool to assist employers with employee performance management.
Regular employee assessment is necessary not only to highlight problem areas that may require additional training or supervision, but also to provide positive feedback and recognition to employees who are performing well and should be performed at least quarterly. The employee evaluation form can be a motivational tool if used constructively. It can also be used to record verbal warnings for poor performance. Records of regular employee assessment are most important should you be challenged for unfair dismissal for poor performance. It is also an excellent tracking tool if used during a probationary period. For effective employee performance management the criteria on the employee evaluation form must be compiled according to the job description. For every performance factor listed, you should provide specific guidelines for judging, tailored to your company. For example: Skills - Handling objectives, customer complaints, add-on selling - For a Salesperson Skills - Maintaining class discipline, eliciting student participation - For a Teacher A mark for unacceptable or poor performance on the employee evaluation form must be accompanied by a comment and the assessment must be done by the manager or supervisor of the employee.
