Affidavit of Residence

You may need an affidavit of residence in addition to other supporting documents to prove residency at an address.

Presenting a utility bill in your name or insurance policy on the dwelling may prove that you are the landlord or owner but does not necessarily prove that you live there.

Conversely, as a tenant you may not have any official accounts in your name linking you to a property or dwelling.

The requirement therefore, by a local school for example, would be to ask for an Affidavit i.e. a statement made under oath or attested to under penalty of perjury to certify your residency.

Within your affidavit you can then refer to and annex (attach) other supporting documents.

For ease of use we've also compiled an Affidavit of Domicile to be used when certifying the address of a person at the time of death. It is important to state your full residential address on both these forms.

If you are not the parent of any children listed, you could add the following:
